This paper presents a review of literature on human error, in the context of human reliability, and its importance in processes in industry and services. The aim of this paper is to present the state of the art on the subject, lead authors, models and methodologies used in the data analysis. These structures include models and taxonomies. The search was performed in databases like EBSCO and ScienceDirect among others. The results are shown in two tables containing the studies that have identified human errors that contribute to accidents and incidents in different areas such as aviation, land and sea transportation, manufacturing industry, nuclear plants, systems industrial electricity, medical services and drinking water systems; further studies have identified human errors that contribute to defects or failures in processes or products in the context of the manufacturing industry with a focus on reliability and quality approach, also in the military industrial occur. The study of human error has been approached from three different approaches: engineering, cognitive psychology and cognitive ergonomics.
